The animation shows an hypothetical patient’s journey  through the different health care organizations. After a screening with an ematologic exam and the diagnosis, the patient has to move to the diabetes centre for a first evaluation. a care plan and then a therapy. At this point the patient has to assume the drugs prescribed and  he has to monitor the blood glucose and send the data to the telemedicine system. Besides the diabetologic visit the patient will have to carried out different specialistic visits and exams to prevent possible complications. The red arrows track represents the steps that the patient follows through the different services.
The patient is forced to move to different location to have access to the services he needs.  The goal is to optimize this flow and to make the data exchange and the data recording of the patient chart easier.
